The group agrees to have adult supervision of any and all children under the age of 18 at all times.
For any sports play, the group is required to provide proof of insurance. The undersigned represents that the group has general liability insurance with coverage limits of 1,000,000.00 in effect as of the date of the activity. The group agrees to name the church as an additional insured on its general liability insurance policy and shall provide proof of such additional insurance coverage to the church prior to the date of the activity.
If the Gym is available on requested date(s) and time(s), a reservation form must be completed (with staff), and payment must be received at least two weeks prior to the event.
Staff will contact you within 3-5 business days regarding availability and next steps.
